Patient Bill of Rights

Introduction
Heir Health respects your rights as a patient. This document summarizes the rights you have when receiving telehealth services through the Service operated by Heir Health LLC. These rights are protected under applicable federal and state laws, our Terms and Conditions, our Telehealth Consent, and our Privacy Policy.
Nothing in this Bill of Rights replaces, modifies, or limits any of those documents. If there is any conflict between this Bill of Rights and another document, the more specific document controls.
Your Rights as a Patient
1. Right to Considerate and Respectful Care
You have the right to receive care that is considerate, respectful, and free from discrimination based on race, color, national origin, age, religion, sex, sexual orientation, gender identity, disability, or any other characteristic protected by law.
2. Right to Informed Consent
You have the right to receive clear, understandable information about your condition, proposed treatment, alternatives, and the risks and benefits of each, so that you can make informed decisions about your care.
3. Right to Refuse Treatment
You have the right to refuse any treatment recommended by your Provider, to the extent permitted by law, and to be informed of the medical consequences of refusing treatment. You may also withdraw your consent to telehealth at any time, as described in the Telehealth Consent.
4. Right to Choose Your Provider
You have the right to obtain medical services from a healthcare provider other than the Providers available through the Service. Use of the Service is voluntary.
5. Right to Choose Your Pharmacy
If a Provider determines a prescription is appropriate for you, you have the right to have that prescription filled at any pharmacy of your choice, not only the Pharmacies that partner with Heir Health. To request a transfer to a different pharmacy, contact us at support@heirhealth.co.
6. Right to Privacy and Confidentiality
You have the right to have your medical information kept private and confidential, in accordance with HIPAA (where applicable), state privacy laws, and the Privacy Policy and Notice of Privacy Practices that apply to your care.
7. Right to Access Your Health Information
You have the right to access, copy, correct, and request deletion of your health information, subject to applicable law. To exercise these rights with respect to your medical record, contact your Medical Group through the patient portal. To exercise these rights with respect to other personal information held by Heir Health, contact us at privacy@heirhealth.co.
8. Right to Be Informed of Financial Responsibility
You have the right to be informed in advance of the costs of services and products before you agree to receive them. You also have the right to know that Heir Health and the Medical Groups are not enrolled with Medicare, Medicaid, or any commercial insurance for the services provided through the Service. You are responsible for the full cost of services and products on a cash basis.
9. Right to Know Your Provider's Credentials
You have the right to know the name and credentials of any Provider who delivers care to you through the Service. If you have a concern about a Provider, you may contact the Medical Board in the state where the Provider is licensed.
10. Right to Emergency Care Information
You have the right to be informed that the Service is not appropriate for medical emergencies. If you are experiencing an emergency, call 911 or go to the nearest emergency room. If you are experiencing a life-threatening mental health crisis such as suicidal ideation, call 911 or the 988 Suicide & Crisis Lifeline.
11. Right to File a Complaint
You have the right to file a complaint about the care you receive through the Service. You may submit a complaint to:
- Heir Health Customer Support: support@heirhealth.co
- The Medical Board in your state: for complaints about a Provider's medical care
- The State Attorney General's office in your state: for complaints about consumer rights, including privacy
Filing a complaint will not affect the care you receive.
12. Right to Continuity of Care
You have the right to receive a summary of your care, including any prescriptions and treatment plans, to share with another healthcare provider of your choice. You are responsible for arranging follow-up care with your primary care provider or other personal physician.
13. Right to Cancel Subscriptions
If you are enrolled in a subscription product or service, you have the right to cancel at any time through your online account under "My Subscriptions," or by contacting support@heirhealth.co. Cancellation takes effect at the end of the current subscription period.
14. Right to Withdraw Consent
You have the right to withdraw your consent to the use of telehealth at any time, without prejudice to any future care. Any withdrawal of consent will not affect actions taken in reliance on your prior consent before your withdrawal was received.
15. Right to Be Free From Discrimination Based on Source of Payment
You have the right not to be discriminated against based on your source of payment, including the fact that you are paying out-of-pocket.
